This procedure includes the following:

* The removal of the deceased from the place of death and transporting him/her to our facility within 40 miles.

* Storage.

*Obtaining a signed death certificate from attending physician or county coroner (coroner’s fee is not included).

* Preparation of disposition permit an other necessary documents.

*Filing of death certificate and disposition permit at the county health  department.

* A cremation container.

* Crematory fees for cremation.

* Standard plastic urn.

* Transfer of cremated remains back to our facility to be released to family.

* 250 Ibs maximum body weight.

Direct Burial:----------------------------$ 950.00

* The removal of the deceased from place of death and transporting him/ her to our facility within 40 miles.

* Storage.

*Obtaining a signed death certificate from attending physician or county coroner (coroner’s fee is not included).

* Preparation of disposition permit an other necessary documents.

*Filing of death certificate and disposition permit at the county health department.

* A minimum fiberboard clothed blue or grey Casket.

* Casketing of remains.

*Transfer of casketed remains to designated cemetery  within 40 miles from our facility (mini van used). It is the responsibility of the family to finalize with selected cemetery prior to transfer of casketed remains.
